Thank you for choosing Mehr Eye Center, please note the following points in order to use your basic and supplementary insurance:
- It is mandatory to bring patient identification documents (national card or ID card) to issue a referral letter.
- It is mandatory to bring an ID card for single women over 15 years old (who use insurance covered by their parents).
- It is mandatory to bring an employment form for single boys over 18 years old (who use insurance covered by their parents).
- For surgeries and inpatient services, the center has a contract with basic insurances (social security and medical services). However, for refractive error correction procedures (LASIK), the center does not have a contract with basic insurances (social security and medical services).
- For refractive error correction surgeries (LASIK, PRK, FemtoLASIK and FemtoSmile), the center has a contract with Sepah Bank and Iran Copper Industries insurance.
- To cover supplementary insurance for surgical procedures other than cataract and pterygium with graft, confirmation from a doctor trusted by insurance companies is required.
- Dear patients who are candidates for blepharoptosis surgery and whose basic insurance is social security, should go to the Medical Documents Department of the Social Security Organization located in Tabriz, Chaykenar, corner of Dimashqeeh Alley, to obtain confirmation from a trusted doctor, with perimetry and an insurance expert referral form in hand.
- Blepharoplasty is not covered by any insurance.
